You can complete forms on a client’s behalf in two ways.
- Scenario 1: Completing Forms When a Client Has Not Filled Them Out
- Scenario 2: Completing Forms After Client Grants Permission
📍 Note: You can only complete forms that do not require the client’s signature.
Scenario 1: Completing Forms When a Client Has Not Filled Them Out
If a client arrives for an appointment without completing their forms, you can fill them out for them if no signature is required.
Steps to Complete the Form
Go to My Practice > Forms & Waivers.
Locate the form you want to complete.
Click the More Options menu (3 dots) > Complete for client > [Select Client].

Complete the form exactly as the client would.
Forms completed this way will include your digital footprint and indicate that you completed them.
Click Save & Submit once finished.
Viewing Completed Forms
Both you and the client can view the completed form in the Forms & Waivers section of the client’s file.
Scenario 2: Completing Forms After Client Grants Permission
If a client has trouble completing their form, they can grant you permission to finish it on their behalf.
How Clients Grant Permission
The client goes to Forms in their Client Portal.
They locate the form they want you to complete.

They click More Options menu (3 dots) > Allow my practitioner to complete.

Steps to Complete the Form After Permission
Go to My Clients > [Select client] > Forms & Waivers.
Locate the form and click Complete.
Fill out the form and submit it.
