You can access Practice Better's library of pre-built forms and waivers to add to your portal and customize for your practice. Templates save you the work of building a form from scratch, so you can get a polished intake form, waiver, or questionnaire in front of clients in minutes.

Before You Begin
This article covers form templates specifically. The Practice Better template library also includes note templates and complete program templates. For the full library experience, including filtering by resource type and practitioner type, see Using the Templates Library in Practice Better →
If you're building a program, program templates are covered in Adding Content to Program Modules →
📍 Note: Adding a template creates a new form in your Forms & Waivers list, but it doesn't automatically get sent to clients or attached to a service. To learn how to deliver a form once it's ready, see Sending Forms to Clients →
Accessing the Form Templates Library
- Go to My Practice > Forms & Waivers.
- Click Create form near the top-right.
- Select Start beside the option to Use a template.
Adding a Template to Your Portal
- Select a form template from the list to Add and customize. You can also choose to View the template first to see its sections and formatting before deciding to use it.
- After previewing the form, click Add Template to add this resource to your portal.
- Choose what to do next: click Done to continue adding templates from the library, click View my forms to go to your Forms & Waivers list, or click Edit to open the Form editor right away.
💡 A form template is added to your portal as an independent copy that is live and ready to send. We recommend reviewing and tailoring the content to your practice before sending it to clients, since templates may contain placeholders for your personal information.
Using a Template Code
If you received a template code from a colleague using Practice Better, click the Unlock Templates button from the Template library page.
Enter the access code in the field provided, then click Submit.
Once submitted, the unlocked template appears in a new Unlocked Templates section in the library. Click it to open and save a copy to your portal, the same way you would add any other template.
