Form Response Summary Report
This report requires a subscription to the Starter Plan or higher.
The Form Response Summary Report allows you to view and analyze multiple submissions for a given form. From your Forms & Waivers page, click the More Options button next to a form and select View responses from the options provided:
Alternatively, you can go directly to your My Practice > Reports section and select the Form Response Summary report. Select a form and click Submit:
Responses will be listed as they are ordered on your form. You can quickly jump to a specific question(s) by selected it from the View all questions picklist:
Analyzing Different Form Versions
Every time you save a form a new version is created. You may notice the following prompt if you've made modifications to your form between client submissions:
This could mean you've added (or removed) questions, changed the question order or question types.
We'll attempt to associate responses to questions based on each question's unique ID; however, it is possible that some responses may be omitted from the report if we're unable to associate answers to the questions included in the version of the form displayed.
Responses to certain question types may include an accompanying chart or word cloud (short response questions). Click on a specific section of a chart to view individual client responses:
You can also click on the expand/collapse button in the top-right corner of each question to view and filter responses:
Double-click on a response to open a client's complete submission.
The Form Response Summary Report can be downloaded in spreadsheet format by clicking Download report in the top-right corner of the screen:
Viewing & Annotating Individual Submissions
You can view an individual form by going to your client's Forms & Waivers section.
Select Annotate to review and add notes to your client's submission:
Click Download as.. to download a copy of your client's form as either a PDF or and Excel spreadsheet
