Practice Better's Unsigned & Incomplete Notes report gives you a practice-wide view of every session note that hasn't been signed yet, so nothing falls through the cracks before an audit, a supervision review, or billing. This article covers how to open and filter the report, what the Cosigned column tells you, and how to move a note from in progress to signed.

Before You Begin
This report tracks three different note states, that each mean different things:
- Completed: Marking a note "Completed" only updates its status; it doesn't lock the note.
- Signing a note locks it as read only, automatically marks it Completed, and locks its sharing settings.
- Cosigning is a separate, optional second signature that a teammate can add after a note is signed; it doesn't change the note's completed status, and Practice Better doesn't require a cosignature for a note to count as finished.
This report only reflects accurate results if your team has been marking notes' statuses as they go. If a note is left in In Progress status, it won't show as completed or signed here, even if the documentation itself is considered finished.
To see another team member's notes in this report, you need both report access (the Organization: Reports permission) and access to that team member's client notes. A team owner can manage team member access under My Team > [Team Member] > Edit.

Opening the Unsigned & Incomplete Notes Report
- Go to My Practice > Reports.
- Click Unsigned & Incomplete Notes under the Notes section.
Filtering the Report
On the left side of the report, you can filter by:
- Date created: narrow the report to notes created within a specific period.
- Practitioner: view notes for yourself or another team member you have access to.
- Note status: select In Progress, Completed, or Completed & Signed.
💡 If you're prepping for an audit or a supervision review, filtering by practitioner and status together is the fastest way to find exactly which notes still need attention.
Understanding the Cosigned Column
The report includes a Cosigned column, both in the portal and in the CSV export, so you can see at a glance which signed notes still need a teammate's cosignature and which are fully cosigned. This column reflects cosigning activity only; it has no effect on a note's completed or signed status.
Downloading the Report
Click the Download button near the top right of the report to export the results as a CSV or Excel file. The exported file includes the same columns you see in the portal, including the Cosigned column.
Completing and Signing Notes from the Report
The report shows you which notes need attention, but you'll complete or sign each note from the client's record.
To mark a note completed:
- Go to My Clients > [Client's Name] and open the Notes section of their client record.
- Click the More Options menu (3 dots) beside the note.
- Select Mark as completed.
To sign a note:
- Click the note to open it.
- Click Save in the top right corner.
- Select Sign and save from the dropdown menu.
⚠️ Important: Signing a note is permanent. Once a note is signed, it can't be unsigned or edited.
