You can upload existing PDF forms and worksheets to Practice Better for your clients to complete.
Go to My Practice > Forms & Waivers from the top navigation menu.
If this is your first Form, you will see a button to Create Form with options to create your Form. You may also explore the available Form templates by clicking Create from template. Learn more.
Select Upload form or worksheet from the options provided here:
If you have an existing Form in your account, you won't see the green Create form button.
You can upload new Forms by clicking the Fast Action Button in the bottom-right corner, then selecting Upload form or worksheet from the options provided:
You can send Forms to clients manually, include them in Automations, and add them to your Program modules.
