When a client completes a form, you can pull their answers, along with any private notes you've added, straight into a new session note. This creates a one-time snapshot of that form, either in a blank note or a note template you choose.

Before You Begin
- You need at least one completed form in the client's Forms & Waivers section. Creating a Note from a form is only available for forms built with Practice Better's Form Builder; it isn't available for PDF Form Uploads.
- Each time you use Create Notes by the completed form’s More Options menu, you're building a brand new note. There's no option to add a form's content into a note you already started, so if you're documenting a series of sessions, plan to create a separate note for each one.
- This is a different workflow from Populating Form Answers into a Note. That approach maps your form's questions to fields in the client's file (like Medical History), then pulls the current file data into any note using placeholders, staying in sync as the file updates. Creating a Note from a Completed Form
- Go to your client's Forms & Waivers section.
- On the completed form, click the More Options menu (3 dots).
- Select Create Notes.
Choosing What to Include
In the Create Notes pop-up, select the questions, answers, and aggregates you'd like to pull into your note. Any question your client answered will be marked as such.
Aggregates are the grand totals and averages calculated from Grid, Numeric, or Scale questions on the form. Learn more about Adding Grand Totals and Averages to Your Forms →
💡 Tip: Use All questions, Questions with answers, or Unselect all at the bottom of the pop-up to quickly adjust your selection instead of clicking through each item one by one.
If you added any private annotations to the form, you can select those too so they carry over into the note alongside your client's answers.
Once you're happy with your selections, click Create. This opens a new, fully editable session note with your selected content already in place, ready for you to review, adjust, and save.
Importing into a Note Template
By default, Create Notes builds a plain, empty note. If you'd rather start from a note template, select it from the pick list in the Create Notes pop-up.
- When building a note template, you can control exactly where the form content lands by adding a %COMPLETED_FORM% placeholder. This is the same placeholder listed as Completed Form Answers in the template's placeholder picker.
- If your template doesn't include this placeholder, the form content is added to the bottom of the first available text section instead.
