Practice Better's form reports let you review, analyze, and download how clients have responded to your forms, whether you're looking at one submission or comparing trends across many. This article covers the Form Response Summary Report, the Individual Form Response Summary Report, and viewing or annotating a single form submission.

Before You Begin
- Viewing a form response helps you review and analyze what a client submitted. It doesn't move that data into the client's chart. If you want form answers to populate fields like Personal Information, Medical History, or Diet & Lifestyle automatically, set up a Form Mapping instead. - The Form Response Summary Report and the Individual Form Response Summary Report (Compare Form) require a subscription to the Starter Plan or higher. Annotating an individual client's form response is available on every plan, including Sprout.
- For the best experience, view the Form Response Summary Report and Individual Form Response Summary Report on a laptop or desktop PC. These reports can't be viewed on phones or small tablets.
Viewing the Form Response Summary Report
The Form Response Summary Report lets you view and analyze every submission for a given form at once, so you can see how your clients are responding as a group.
You can get there two ways:
- From your Forms & Waivers page, click the More Options menu (3 dots) next to a form and select View responses.
- Or, go directly to My Practice > Reports, select the Form Response Summary report. With the report open, choose a form, and click Submit.
Responses are listed in the same order as the questions on your form. Use the View all questions picklist to jump straight to a specific question.
Analyzing Different Form Versions
Every time you save changes to a form, a new version is created. If you've modified a form's questions, order, or question types between client submissions, you may see a prompt like this when viewing the report.
We match responses to questions using each question's unique ID behind the scenes. Occasionally, a response may be left out of the report if we can't confidently match it to a question in the version you're viewing.
Viewing an Individual Client's Response Trends (Compare Form)
The Individual Form Response Summary Report shows you how one client's answers to a form have changed across multiple submissions, so you can track their progress over time.
To generate this report:
- Go to the Forms & Waivers section of the client's record.
- Click the More Options menu (3 dots) next to a completed form.
- Select Compare form.
Visualizing Responses
Certain question types display an accompanying chart or word cloud (for short response questions), so trends are easy to spot at a glance.
- Click a specific section of a chart to view the individual client responses behind it.
- Click the expand or collapse button in the top-right corner of any question to view and filter its responses.
- Double-click any individual response to open that client's complete submission.
Downloading the Form Response Summary Report
To download all responses to a form as a spreadsheet, click Download report in the top-right corner of the screen.
Viewing an Individual Submission
To view a single client's completed form, go to their Forms & Waivers section in their client record.

Annotate a form: click Annotate next to the completed form to review it and add your own private notes. This opens the same annotation view covered in Adding Private Notes to Completed Forms → where you can add summary notes, notes on individual questions, and pin notes for quick reference.
📍 Note: Annotate isn't available for uploaded forms. For these, you can View or Download the submission from the client's record, but you can't add private notes directly to them.

Download a submission: click Download as... to save a copy of the client's form as a PDF or Excel spreadsheet.
