Practice Better offers three flexible ways to complete and submit forms: through your Client Portal (with progress saving), directly from email notifications, or by uploading completed downloaded forms. This ensures you can provide important health information to your practitioner in whatever way works best for your schedule.

Access Forms Through the Client Portal
When you have full access to your Client Portal, you have more control over when and how you work on your forms.
- Click on the Form's title in the Pending Forms widget.
- Or go to Forms from the client portal menu.
- Or go to Forms from the client portal menu.
- Click the Complete button on the specific form you want to work on.
Access Forms Directly from Email
For ultimate convenience, Practice Better allows you to complete forms without even logging into your portal.
Quick Email Access
When you receive a Practice Better forms notification email, simply click on the link to the form.
- If you have an active client portal, you’ll be redirected to a login page before completing your Form.
- If you don’t have a client portal, the fillable Form will open in your browser.
This direct access means you can complete important health forms immediately, whether you're at home, work, or anywhere with internet access.
Completing Your Form
Take your time filling in all the required fields. Your practitioner understands that thoughtful responses lead to better care, so there's no need to rush through this important step.
Saving Your Progress
Life happens, and sometimes you need to step away before finishing:
- Click the Autosave icon near the top-right.
- Confirm that your progress has been saved recently to preserve your work.
You can return anytime to pick up exactly where you left off - Practice Better keeps track of everything for you.
Submitting Your Completed Form
- Fill in all the required fields and review your responses.
- Click the Save & Submit button on the bottom right of the screen.
Your practitioner will receive your completed form and can begin incorporating this valuable information into your personalized care plan.
Submit Downloaded Forms and Worksheets
Some practitioners prefer to send forms for you to download, complete offline, and then upload them back to the system.
Downloading Your Form
- Go to Forms from the client portal menu.
- Click the Complete button on the specific form you want to work on.
- Click the Download form button to save the document to your local device.
- Open the downloaded form and complete it.
💡 Use a PDF viewer or text document program to open and complete your Form. If available, print your form and fill it out by hand.
Uploading Your Completed Files
- Go to Forms from the client portal menu and click the Complete button on the form you need to upload.
- On the subsequent page, click Attach File(s) to browse and select the completed files from your computer.
- Click Attach more if you need to include additional worksheets, forms, or supporting documents.
- Once all your files are attached, click Submit to send everything to your practitioner.
- You'll see a confirmation pop-up asking you to verify your submission.
- Click Submit again on this pop-up screen to ensure your forms are delivered to your care team.
Practice Better accepts various file formats, making it easy to submit whatever works best for you. This flexibility ensures you can provide your practitioner with a complete picture of your health information in a single, organized submission.
