Using the Automatic Form Builder
- Navigate to My Practice > Forms & Waivers.
- Click Create New Form in the top-right.
- Select the Automatic Form Builder option.
- Confirm the Form purpose, your practice type, and level of detail.
- Provide a specific description of the form you want to generate, including any specific sections or questions you want to insert.
- Click Preview to create your customized form.
- Click Build Form to save the generated form in your list of Forms & Waivers.
