In order to have a client's form answers automatically populate into a new note, you will have to map and import your client's form data into their file, and then use placeholders in your note template to draw information from their file into the note.
How to populate form data into a note
1. Set up a mapping and import the form data into your client's file
You can map form data into the follow sections of a client's file:
- Basic Information
- Medical History
- Diet & Lifestyle
Once you have set up a mapping for your form, you will have to import your client's answers into their file.

2. Set up placeholders in your Note Template
Once your client's form data has been imported into their file, you can then use placeholders that will draw information from their file. Placeholders can be used for the following sections of their file:
- Basic Information
- Medical History
- Diet & Lifestyle
- (Current) Measurements & Vitals
- Answers from a completed form
To add placeholders to your note template, click the Add Placeholder Text button in the top-left corner of the text editor:
Once you've selected a placeholder, it will appear in the editor like this:
You can apply further formatting to the template or add it to a table within the editor to meet your requirements.
When you're done, you can create the note for your client.

How to import a completed form directly into a note
You also have the option to import a client's completed form and your private annotations into a new note. With this method, you will have to use the %COMPLETED_FORM% placeholder in your note template:
If the %COMPLETED_FORM% placeholder is not present in the template, the completed form will then be imported at the bottom of the first available text section in the note.
